Then if you draft a fourth outfielder, does he have to be on the bench?

Yes, we draft all 16 players, so you may have 5 or 6 OFs but some will be on your bench.

The real question is, what happens if you only draft 3 IF or no C, are you kicked out of the league?

I think we just disallow illegal picks - you can have no more than 4 non-starting players.  For example, if someone has 7 OFs and tries to draft another one, they have to pick someone else (since the 4 extras would fill up the bench).  The same applies if we're at the end and the person needs another RP - that would be the only legal pick, and if he tried to take an OF, we'd make him repick while mocking him mercilessly.
